“Are my school forms up to date?”

The vast majority of “summer forms” that we once collected via paper were incorporated into the online enrollment process, which began in February. Medical forms and a variety of other policies were filled out at that time and became part of your student’s records when you signed your contract. If you would like to review those policies and see how you responded to them during online enrollment, they can be found in the School Policies section of the Household Profile in the Parent Portal. There are a few state-required, non-electronic forms below that we need to collect in person.

Household Profile Updates

Click the Veracross logo to the right to log into the Parent Portal. Please double-check your Household Profile Update to make sure we have all the necessary information. We specifically need to know that your student’s Medical Information within the HPU is complete, with insurance information and your family doctor listed. You may also review the electronic School Policies you were asked to complete during the enrollment process.

All events are entered into the Veracross calendars, which can be accessed via the Parent Portal. You can subscribe to the calendar feeds that most apply to your family.

Immunization Information Details

The official CIS form must be completed and signed by a parent or guardian (it does not require a doctor’s signature). Parents can now go online to view and print their child’s Certificate of Immunization status here. Please note: Faxes or printouts from a doctor’s office listing immunizations are not acceptable. See this link for the complete state requirements for the 2018-19 school year.

Athletic Information Details

All students in grades 7-12 are eligible to participate on interscholastic teams provided they have turned in their proper athletic and medical forms, including immunization forms. Students may not participate in interscholastic sports (including practices) until the forms are received. All new students entering 7th-12th are required to have a full physical examination and submit the WIAA Required Physical Pre-participation Report Form whether or not they intend to play an interscholastic sport. Please have your family physician complete this required WIAA physical form. Please note: Medical providers must use the WIAA forms; signatures on blank forms with an attached print-out are not acceptable.

Returning students entering into 7th, 9th, and 11th grades must submit the WIAA Required Physical Pre-participation Report Form (based on a full physical examination) whether or not they play an interscholastic sport.

Returning students entering 8th, 10th, and 12th grades are not required by the school to have a full physical examination unless their last physical examination has expired. Physical exams are good for two years from the date of the examination.
