Daily Health Assessment Required for Campus Access

Until further notice, all students, employees and visitors to the Charles Wright Academy campus must complete a daily health assessment, and you must sign in and out with an authorized representative in an appropriate area of operations on campus.

COVID-19 HEALTH ASSESSMENT

  • Fever (100.4 F)

  • Chills

  • Cough

  • Shortness of breath or difficulty breathing

  • Fatigue

  • Muscle or body aches

  • Headache

  • Recent loss of taste or smell

  • Sore throat

  • Congestion or runny nose

  • Nausea or vomiting

  • Diarrhea

2) Has anyone in your household had a positive COVID-19 test in the past 10 days?

3) Has anyone in your household been in close contact with anyone with confirmed COVID-19?

4) Within the past 14 days, has a public health or medical professional told anyone in your household to self-monitor, isolate or quarantine because of concerns about COVID-19 infection?

If the answer to ANY of the questions is YES,

you may not come to campus and should remain at or return to your home, immediately. You may need to complete a longer quarantine or seek medical certification in order to return to campus at a later date. Please see Department of Health guidelines for more information and what to do if you answered yes to any questions in the Health Screening. Employees should notify their supervisor and/or Human Resources to let them know.

These assessment questions are subject to change at any time.

If the answer to all the questions is NO,

you can certify such upon signing in at a designated reception area when you arrive on campus. While on campus, you must follow all hand washing, social distancing, and mask guidelines as required for Washington State.
